In homeopathy and traditional medical systems, human physiology and temperament are often categorized into four primary constitutions. Each of these constitutions is associated with distinct physical, emotional, and metabolic characteristics that influence an individual's health tendencies and responses to treatments.

Samuel Hahnemann, the founder of homeopathy, did not explicitly categorize patients into the traditional four constitutions—Sanguine, Phlegmatic, Choleric, and Melancholic—as commonly used in classical medicine and temperamental theories. However, he did recognize constitutional tendencies and chronic disease predispositions in his writings, particularly in The Chronic Diseases and The Organon of Medicine.

Hahnemann’s View on Constitutions

Hahnemann emphasized that individuals have unique susceptibilities based on their inherited and acquired health tendencies. Instead of using the four humoral constitutions, he introduced the concept of miasms, which are deep-seated disease tendencies that affect a person’s overall health. He believed that chronic diseases stem from inherited or acquired miasmatic influences, such as Psora, Syphilis, and Sycosis, which interact with a person’s constitution.

Relation to the Four Constitutions

While Hahnemann did not directly discuss the four constitutions, later homeopaths integrated these classical temperaments into homeopathic practice. They recognized that different constitutions respond uniquely to remedies and have specific predispositions toward certain ailments:

Sanguine individuals (energetic, warm, and excitable) were often linked to Psoric tendencies.
Phlegmatic individuals (slow metabolism, fluid retention, and lethargy) showed tendencies toward Sycosis.
Choleric individuals (prone to anger, inflammation, and overactivity) were sometimes associated with Syphilitic tendencies.
Melancholic individuals (introspective, anxious, and cold) were linked to deeper constitutional imbalances, including Psora or Syphilis.

Sanguine Constitution

The Sanguine constitution is characterized by a lively, energetic, and social disposition. Individuals with this constitution typically have a strong circulatory system, warm skin, and a robust digestive function. They tend to have a fast metabolism, which allows them to process food efficiently and maintain high energy levels. However, their tendency toward impulsivity and excitement can lead to excessive stimulation of the nervous system, predisposing them to anxiety, restlessness, and cardiovascular issues such as hypertension. Homeopathic remedies like Phosphorus and Pulsatilla are often used to balance their heightened nervous and emotional energy while supporting circulatory health.

Phlegmatic Constitution

The Phlegmatic constitution is associated with a calm, slow-moving, and patient nature. These individuals often have a slow metabolism, leading to a tendency toward weight gain, fluid retention, and sluggish digestion. Their immune systems may also be weaker, making them more susceptible to respiratory infections, allergies, and hormonal imbalances such as hypothyroidism. Due to their passive nature, they may struggle with motivation and physical activity, requiring stimulation to maintain balance. Homeopathic remedies such as Calcarea Carbonica and Silicea can help improve metabolic function, enhance vitality, and regulate mucus production in those with phlegmatic tendencies.

Choleric Constitution

The Choleric constitution is defined by a fiery, determined, and goal-oriented personality. These individuals often have a fast metabolism and a tendency toward inflammation, making them more prone to hyperacidity, liver imbalances, and conditions such as gastritis or autoimmune disorders. Their high energy and ambition can lead to chronic stress, which affects the adrenal glands and may contribute to conditions like hypertension and anxiety. Homeopathic remedies such as Nux Vomica and Sulphur are often recommended to support liver function, improve digestion, and regulate inflammatory responses in individuals with a choleric constitution.

Melancholic Constitution

The Melancholic constitution is associated with deep thinkers who are often introverted, detail-oriented, and prone to worry. They typically have a sensitive nervous system and a slower metabolism, making them more susceptible to digestive issues, chronic fatigue, and depression. Melancholic individuals often experience poor circulation, cold extremities, and an increased tendency toward constipation and musculoskeletal pain. They may also have a harder time adapting to stress, leading to nervous exhaustion. Homeopathic remedies like Ignatia and Sepia can help balance their emotional well-being, support digestion, and enhance their overall resilience to stress.

Hahnemann’s Emphasis on Individualization

Rather than fitting patients into rigid constitutional types, Hahnemann stressed individualization, meaning that each patient must be treated as a unique case, considering their physical, emotional, and mental symptoms. His philosophy was that true healing does not come from classifying patients into broad categories but by selecting a remedy based on their totality of symptoms, including temperament, personality, and environmental influences.
