Understanding Prolactin: A Hormone for Both Men and Women

Prolactin, a hormone primarily produced by the anterior pituitary gland, plays vital roles in the body. While it's most commonly associated with stimulating milk production in women, prolactin also influences reproductive health, immune function, and metabolic processes in both men and women. Dysregulated prolactin levels can lead to various symptoms and health concerns.

Prolactin in Women

In women, prolactin levels naturally fluctuate, especially during pregnancy and breastfeeding when levels are highest. Outside these states, prolactin helps regulate reproductive health. Elevated levels in non-pregnant women can cause menstrual irregularities, infertility, and symptoms like breast tenderness. Low levels are rare but may impair lactation and reproductive functions.

Prolactin in Men

Though less discussed, prolactin also has significant roles in men, contributing to testosterone production and sperm development. Elevated prolactin levels can lead to reduced libido, erectile dysfunction, gynecomastia (breast enlargement), and infertility. Low levels of prolactin in men are less well understood but may impact the immune system and overall hormonal balance.

Symptoms of High Prolactin Levels (Hyperprolactinemia)

In Women:
- Irregular or absent menstrual periods
- Infertility
- Galactorrhea (milk production unrelated to childbirth)
- Breast tenderness
In Men:
- Decreased libido
- Erectile dysfunction
- Gynecomastia
- Infertility
Both:
- Headaches (due to pituitary tumors causing elevated prolactin)
- Vision problems (if caused by a pituitary tumor)

Symptoms of Low Prolactin Levels

Low prolactin levels are uncommon but may result in:

Inability to lactate in breastfeeding women.
Possible immune system dysregulation in both genders.
Reduced reproductive efficiency in men.

Causes of High Prolactin Levels

Physiological Causes:
- Pregnancy and breastfeeding
- Stress
- Exercise
Pathological Causes:
- Prolactinomas (pituitary tumors)
- Hypothyroidism (low thyroid hormones can stimulate prolactin release)
- Kidney or liver disease (which impair prolactin clearance)
Medications:
- Antipsychotics (e.g., risperidone, haloperidol)
- Antidepressants
- Hormonal contraceptives
- Some antihypertensives
Other Factors:
- Chest wall injury or stimulation
- Estrogen therapy

Causes of Low Prolactin Levels

Pituitary Damage:
- Injury, surgery, or conditions like Sheehan’s syndrome can impair prolactin production.
Dopaminergic Overactivity:
- Dopamine inhibits prolactin; conditions or drugs that increase dopamine activity can lower prolactin levels.

When to Test Prolactin Levels

Testing prolactin levels may be recommended if:

You experience symptoms of hormonal imbalance (e.g., menstrual irregularities, libido issues).
Infertility investigations are underway.
A pituitary disorder is suspected.
Unexplained symptoms like headaches or vision problems occur alongside hormonal signs.

Managing Prolactin Imbalances

For High Prolactin Levels:
- Medication: Dopamine agonists (e.g., cabergoline, bromocriptine) to suppress prolactin production.
- Surgery or Radiation: In cases of large pituitary tumors unresponsive to medication.
- Thyroid Therapy: Treat underlying hypothyroidism.
For Low Prolactin Levels:
- Address underlying causes, such as restoring pituitary function or balancing dopamine activity.

Prolactin is a multifaceted hormone critical to both men’s and women’s health. Regular monitoring and appropriate management of prolactin levels can help prevent reproductive, metabolic, and neurological complications.
